Indeed, some trials have reported improvements in stair climbing power with testosterone administration (8, 14). The 6MWS continued to improve throughout the intervention duration and we do not know whether a longer duration of intervention may have enabled the neuromuscular adaptations needed to translate testosterone-induced muscle mass and strength gains into clinically meaningful functional improvements. Taken together, these findings suggest a likely small benefit of testosterone on mobility in older men with low testosterone levels.
Testosterone plays a key role in muscle mass and connective tissue strength. The locomotive system includes support (bones), mobility and impact absorption (joints and intervertebral disks), drive and control (muscles, nerves), and network (blood vessels). Testosterone replacement therapy (TRT) can have several positive effects on joint health and mobility. Additionally, testosterone aids in the maintenance of bone density, which is essential for joint stability and strength. In recognition of these concerns the TOM study will incorporate several measures to minimize the risks to the participant. Measures of physical function are now the principal outcomes to gauge the efficacy of potential anabolic therapies because of their strong predictive ability for hospitalization , disability and even mortality 6, 7. The DSMB will convene every 6 months and is empowered to discontinue treatment in one or more subjects or halt the study should the occurrence of adverse events so warrant.
Despite the clear anabolic effects of testosterone in older subjects, its administration has not consistently translated into improvements in muscle strength or when studied, physical function or mobility 24, 27–33. Unique aspects of the TOM Trial include selection of men with self-reported as well as objectively demonstrable functional limitations, community-based screening and recruitment, adjustment of testosterone dose to ensure serum testosterone levels in the target range while maintaining blinding, and inclusion of a range of self-reported and performance-based physical function measures as outcomes. Data are reported for men enrolled in the PFT and those who were not, and for all men in TTrials; data are also reported according to baseline walking speed and mobility limitation. Using data from the PFT and the overall TTrials study population, we also aimed to identify whether the effect of testosterone on mobility differed according to baseline walking speed, mobility limitation, or other participant-level factors. The anabolic effects of testosterone on skeletal muscle mass and muscle strength are well recognized, but it is not known whether testosterone improves physical function and mobility or reduces the risk of falls in older men. In summary, testosterone administration in older men with mobility limitation consistently improved self-reported measures of physical function and likely improved mobility, but did not affect fall frequency.
An Institute of Medicine panel concluded that there was insufficient evidence of a beneficial effect of testosterone replacement on physical function and mobility in older men with functional limitations (16). Testosterone-treated men with baseline walking speed ≥1.2 m/sec experienced significantly greater improvements in 6MWD and in PF10 than placebo-treated men. It’s also important to consider the effects of oestrogen, progesterone and testosterone on muscles, which will affect the pelvic floor – and therefore can have an effect on bladder control too. Lower levels of testosterone can lead to decreased muscle tone and further instability of hypermobile joints. Understanding the impact of testosterone on joints/connective tissues/muscle may provide insights for improving the management of hypermobility disorders in everyone. However, testosterone replacement therapy (TRT) can help restore testosterone levels and improve joint health. Low testosterone levels can contribute to joint pain, stiffness, and limited range of motion.
This should prompt a closer look at how hormonal factors can influence symptoms and quality of life for those affected by hypermobility. There is an increasing recognition of hypermobility-related disorders in the UK and a rise in use of hormone replacement therapy for perimenopausal symptoms. Recent research suggests that hormone fluctuations can play a role in variability of hypermobility related symptoms, especially for women. The purpose of this review is to investigate the role of testosterone in each of the systems involved in the locomotive syndrome. The new gameplan for max muscle now—and the rest of your life.
AbbVie (formerly Solvay and Abbott Laboratories) generously provided funding and donated AndroGel and placebo gel. These findings of the PFT are important in the context of the substantial pharmaceutical investment in exploring the application of androgens as function promoting therapies. In 2002 the National Institute on Aging (NIA) requested that the Institute of Medicine (IOM) assess the status of clinical research on testosterone therapy in older men. The number of men reporting falls or seeking medical attention for fall-related injuries during the year on treatment was similar in each treatment group. We assumed the minimal clinically important difference in 6MWD to be 50 meters, based on the information from epidemiologic studies available at the time the trial was designed (25–27). The time X treatment interaction was not statistically significant; thus the apparent fluctuations in PF10 scores over time may be a chance finding.
The observation that testosterone administration increases skeletal muscle mass and maximal voluntary muscle strength (1–9) has led to considerable pharmaceutical interest in applying testosterone as an anabolic therapy to improve physical function and to reduce the burden of disability in older men with mobility limitation. In this initial trial, we are confident that testosterone therapy will increase muscle mass and strength (primary outcome) and propose that this will translate into improvements in physical function (secondary outcomes). In contrast to recent trials of replacement therapy in older men that achieved only marginal increases in testosterone levels, this study aims to restore testosterone to the mid- to high-normal range. Subjects will perform laboratory-based measures of muscle performance, physical function and physical activity at baseline and following 3 and 6 months of treatment. The secondary aim of this study is to test whether testosterone administration mediates improvements in self-reported as well as performance-based measures of physical function, self-reported disability and habitual physical activity. A total of 252 community dwelling individuals aged 65 and older with low testosterone levels and self-reported limitations in mobility and short physical performance battery (SPPB) score between 4 and 9 will be randomized to receive either placebo or testosterone therapy for 6 months. Unlike previous trials, which often used surrogate endpoints such as lean body mass and muscle performance measures, the TTrials included physical function outcomes that were deemed patient-important and of public health significance.

It’s important to consult with a healthcare professional to determine if TRT is suitable for you and to manage any potential risks or side effects. By promoting the production of synovial fluid and maintaining bone density, TRT can alleviate joint discomfort and enhance mobility. Testosterone plays a significant role in joint health and mobility for men. Adhering to the prescribed treatment plan and maintaining a healthy lifestyle, including regular exercise and a balanced diet, can help optimize the benefits of TRT for joint health.
Comparison of change in 6-minute walking distance and PF10 scores between testosterone and… Comparison of change in 6-minute walking distance and PF10 scores in men enrolled… Of 790 TTrials participants, 395 were allocated to testosterone and 395 to placebo; of the 390 participants enrolled in the PFT, 193 were allocated to testosterone and 197 to placebo. Participants were assigned (by minimisation method) to 1% testosterone gel or placebo gel daily for 12 months, with participants and study staff masked to intervention allocation. With a one-year intervention period, the TTrials also are among the longest testosterone trials. By repeated monitoring of testosterone levels and blinded dose adjustments, we were able to raise and maintain testosterone levels in the mid-normal range for healthy young men. The TTrials included men with unequivocally low testosterone levels, measured using LC-MS/MS.
