There is currently little clinical evidence that exogenous androgen treatment will lead to prostate disease,3,31,32 such as benign prostatic hyperplasia or prostate cancer.33,34 In spite of this, androgen administration to men above age 50 requires careful monitoring of the prostate. If the ED is not reversed, which is common,27,28 but the other aspects of hypogonadism are improved, it may be prudent to keep these men on exogenous testosterone as long as there are no contraindications to its use (Table 3).29–31 In our clinic, we treat such patients for 3 months with exogenous testosterone to determine whether there is an effect on either the ED or the clinical signs of hypogonadism (if present). As a result of the increase in circulating estrogens, these men with hyperthyroidism may complain of or present with gynecomastia, spider angiomas, and a decrease in libido.25,26 Treatment of the thyrotoxicosis reverses the symptoms and signs of the disorder. Or does one use the free testosterone level to determine hypogonadism, in which case about 50% of aged men will fall into this category? Does one compare the total testosterone values found in older men with those found in younger men? As men reach the ages of 40–50, natural testosterone levels begin to fall. Waking with an erect penis is a sign of healthy blood and nerve supply to the penis. Nocturnal penile tumescence (NPT), sometimes called morning wood, is a type of random erection. Another review from 2006 specifically looked at the role of testosterone in erections and found that there is also a connection. A 2016 review reiterated that testosterone plays a significant role in sexual function. A random erection is an erection that occurs without sexual stimulation. They may occur when waking up in the morning, due to hormonal fluctuations during the day, or as a result of erectile dysfunction (ED). Fluctuations in testosterone levels throughout the day can contribute to spontaneous erections, though the exact mechanisms underlying this relationship remain a subject of ongoing research. There is no need to medically manage random erections as they are a normal occurrence. If random erections become troublesome or occur with other symptoms, seek medical advice. One key hormone involved is testosterone, which tends to rise during sleep and peaks in the early morning hours. Healthy men typically experience 3–5 erections per night, especially during REM sleep. Morning erections, also called nocturnal penile tumescence, are more than a sign of virility — they’re a real-time update on your vascular, hormonal, neurological, and even mental health. If you’re waking up without a morning erection — and it’s happening more than just occasionally — it could be more than just a fluke. Spontaneous erections are a sign of vascular and hormonal vitality. Combine that with residual testosterone or mild bladder pressure, and your body may react with a full erection—no arousal needed. The penis is filled with soft tissue that needs to be stretched and oxygenated regularly to stay healthy. A random erection in high-stress moments like interviews or presentations. That’s not dysfunction—it’s prime virility. Fluctuations in your T levels throughout the day can lead to spontaneous arousal—even if your mind isn’t turned on. Read on to learn more about the possible causes of random erections, potential links with erectile dysfunction (ED), and how to manage random erections If you’re having difficulty achieving or keeping erections or you have other symptoms of low testosterone, talk with a doctor. While the scrotal patch (Testoderm) requires scrotal shaving weekly and increases DHT levels somewhat beyond the normal range, normal physiologic serum testosterone levels can be obtained.
